Registrations for fall 2012 are being accepted. Deadline is July 31, 2012.

□ Parish Participation Voucher /Or letter from your church stating you are an active member
□ Background Check Form http://www.holyfamilyclemmons.com/Backgroundcheck.pdf
□ Enrollment for Protecting God’s Children or completion form if you have already taken

□ Attend planning meetings
□ Volunteer and involvement in some aspect of the program
□ We ask for parents enrolling to commit for one semester
□ Must be on-site at all times during enrichment activities
